PARENT/GUARDIAN CONSENT, LIABILITY WAIVER, AND RELEASE OF CLAIMS

IMPORTANT: THIS IS A LEGAL DOCUMENT. BY SIGNING BELOW, YOU ARE WAIVING CERTAIN LEGAL RIGHTS, INCLUDING THE RIGHT TO SUE. PLEASE READ CAREFULLY BEFORE SIGNING.

1. Program Overview

Lauren B’s Jr. Beauty Boot Camp (hereinafter referred to as the “Program,” “we,” or “us”) is a mobile, hands-on youth cosmetology workshop providing instruction in hair styling, artificial hair application, makeup, eyelashes and adhesives, hot tools, sewing needles and thread, and other cosmetology practices. The Program operates at various venues in multiple cities and is structured as a drop-off program, meaning minors are left in the care of trained Program staff and instructors.

2. Assumption of Risk

I, the undersigned parent or legal guardian of the minor child named below (“Participant”), acknowledge and understand that participation in the Program involves inherent and foreseeable risks, including but not limited to:

• Use of hot tools (e.g., curling irons, flat irons)

• Use of sharp instruments (e.g., scissors, needles)

• Use of chemicals and adhesives (e.g., hair products, eyelash glue)

• Potential allergic reactions

• Accidental cuts, burns, or other injuries

• Interaction with other children and staff

• Temporary separation from parents or guardians

I voluntarily assume full responsibility for any and all risks of loss, injury, illness, property damage, or death that may result from the Participant’s participation in the Program, whether caused by negligence of the Program, its staff, volunteers, contractors, venue, or otherwise.

3. Release of Liability and Waiver of Claims

To the fullest extent permitted by law, I hereby release, waive, discharge, and hold harmless Lauren B’s Jr. Beauty Boot Camp, its owners, officers, employees, contractors, affiliates, sponsors, volunteers, and all host venues (collectively, the “Released Parties”) from any and all claims, demands, actions, or causes of action of any kind, whether in law or equity, arising out of or related to:

• Personal injury or death

• Property damage or loss

• Allergic reactions

• Burns, cuts, or other injuries

• Emotional distress

• Lost or missing children

• Any claims of negligence

This waiver applies to all claims, known or unknown, foreseen or unforeseen, arising out of participation in the Program.

4. Drop-Off and Supervision Policy

I understand that the Program is a drop-off activity. I voluntarily place my child in the care of Program staff and acknowledge that while reasonable efforts will be made to provide supervision, the Program cannot guarantee constant direct observation or prevention of all incidents. I remain solely responsible for timely pick-up and communication with staff.

5. Emergency Medical Authorization

I authorize the Program and its personnel to provide or obtain emergency medical treatment for the Participant as deemed necessary in their discretion. I agree to be financially responsible for any medical or emergency services rendered.

6. Photography and Media Release

I grant the Program the irrevocable right to photograph, record, and otherwise capture the Participant’s image, likeness, and voice for promotional, educational, or commercial use, without compensation. If I object, I will provide written notice to the Program prior to the workshop.

7. Venue Disclaimer

I acknowledge that the host venue is not affiliated with or responsible for the Program, its staff, or participants. I agree to hold the venue and its employees harmless from all claims, losses, or damages related to the Program.

8. Governing Law and Severability

This Agreement shall be governed by and construed in accordance with the laws of the state in which the Program is held. If any provision of this Agreement is found invalid or unenforceable, the remaining provisions shall remain in full force and effect.

9. Non-Certification Disclaimer

I acknowledge that participation in Lauren B’s Jr. Beauty Boot Camp does not confer any license, certification, or professional qualification in cosmetology. The Program is for educational and recreational purposes only and does not authorize participants to perform cosmetology services professionally.

CANCELLATION & PAYMENT POLICY

1. No Refunds or Transfers. All payments are final and non-refundable. No transfers, credits, or makeup classes are permitted.

2. Schedule Changes. Dates, times, locations, and instructors are subject to change. Notice will be provided.

3. Payment Schedule.

   • Deposit due at registration to reserve a spot.

   • First payment due two (2) weeks before class.

   • Final payment due two (2) days before class.

   • Late or missed payments may result in cancellation without refund.

4. Payment Method. All payments must be made online. All classes are held in person.

NON-COMPETE & NON-SOLICITATION AGREEMENT

1. Non-Competition. For a period of two (2) years following participation in the Program, I shall not create, host, promote, or participate in any program, class, or event that imitates or competes with the Program’s name, concept, format, branding, or curriculum, including youth cosmetology workshops or similar events.

2. Non-Solicitation. I shall not directly or indirectly contact or recruit Program students, families, staff, vendors, or partners for the purpose of offering a similar or competing service.

3. Confidentiality. All Program materials, methods, and information are proprietary and confidential. I shall not copy, record, distribute, or reuse any Program materials without prior written consent.

4. Legal Consequences. Any violation of this Agreement may result in legal action, including injunctive relief, monetary damages, and other remedies available under law.

5. Acknowledgment. I acknowledge and agree that these terms are legally binding and enforceable.

ACKNOWLEDGMENT AND CONSENT

By signing, I acknowledge that I am the parent or legal guardian of the Participant. I have carefully read and fully understand this Parent/Guardian Consent, Liability Waiver, and Release of Claims, as well as the Cancellation & Payment Policy and Non-Compete & Non-Solicitation Agreement. I sign this document voluntarily and with full knowledge of its legal significance.
