Employee/Participant Consent, Liability Waiver & Release of Claims IMPORTANT: This is a legal document. By signing, you give up certain legal rights. Please read carefully. Program Overview Lauren B’s Jr. Beauty Boot Camp (“the Program,” “we,” or “us”) is a traveling, hands-on youth cosmetology program covering hairstyling, artificial hair application, makeup, eyelashes and adhesives, hot tools, sewing needles and thread, and related cosmetology activities. This document applies to all employees, assistants, volunteers, and support staff — whether adult or minor — who work or participate in any Program activities. 1. Assumption of Risk I understand that working or assisting in the Program may involve certain risks, including but not limited to: Use of hot tools (curling irons, flat irons, etc.) Use of sharp objects (scissors, needles, etc.) Use of chemicals and adhesives (hair products, eyelash glue, etc.) Possible allergic reactions Accidental cuts, burns, or other injuries Physical activity and interaction with children, parents, and staff I voluntarily assume full responsibility for any injury, illness, property loss, or other harm that may occur during or after participation in the Program, whether caused by negligence or otherwise. 2. Release of Liability and Waiver of Claims To the fullest extent allowed by law, I release and hold harmless Lauren B’s Jr. Beauty Boot Camp, its owners, officers, employees, contractors, affiliates, sponsors, volunteers, and host venues from all liability, claims, or damages arising out of my participation or employment. This includes but is not limited to: Injuries, illness, or death Allergic reactions Property damage or loss Emotional distress Claims of negligence 3. Roles, Responsibilities & Supervision I understand I am expected to follow all safety protocols, staff instructions, and Program rules at all times. Minors: Will work under staff supervision and are not responsible for independent instruction. Adults: Are responsible for their assigned duties and for maintaining a safe environment. All staff must act professionally and respectfully at all times. 4. Emergency Medical Authorization I authorize the Program and its staff to provide or seek emergency medical treatment if needed. I agree to be financially responsible for any medical costs incurred. 5. Photography & Media Release I grant the Program the right to use my image, likeness, or voice in photos, videos, or promotional materials without compensation. (If I do not agree, I must provide written notice before participation.) 6. Venue Disclaimer I understand that the venue hosting the Program is not responsible for any claims, injuries, or losses related to the Program. I agree to hold the venue harmless. 7. Non-Certification Disclaimer I understand that participation or employment with the Program does not provide any cosmetology license or professional credential. This is an educational and recreational program only. 8. Governing Law and Severability This agreement is governed by the laws of the state where the Program is held. If any section is found invalid, the rest of the agreement remains enforceable. 9. Acknowledgment & Consent I have read and understand this agreement. I sign voluntarily and agree to follow all Program rules and safety guidelines. Adult Employees/Volunteers: Sign below to confirm acceptance. Minor Employees/Volunteers: Parent or legal guardian must also sign to provide consent.
Non-Compete & Non-Solicitation Agreement By enrolling in or participating in Lauren B’s Jr. Beauty Boot Camp (“the Program”), I understand and agree to the following legally binding terms: 1. Non-Compete I agree not to create, host, promote, or participate—directly or indirectly—in any program, class, workshop, event, or business that duplicates, imitates, or competes with the Program. This restriction applies indefinitely and includes but is not limited to: Youth cosmetology boot camps, classes, or workshops Events using similar names, branding, concepts, themes, or activities Any program targeting the same or substantially similar audience with comparable services 2. Non-Solicitation I agree not to: Contact, recruit, or solicit any Program students, participants, families, staff, contractors, venues, or vendors Encourage any third party to do so for the purpose of offering or promoting any similar, competing, or related services. This restriction is indefinite and remains in effect after my participation in the Program ends. 3. Confidentiality All Program materials, methods, ideas, lesson plans, activities, and other proprietary information are the exclusive property of Lauren B’s Jr. Beauty Boot Camp. I will not copy, record, share, disclose, or use any of these materials for any purpose outside the Program without prior written permission. 4. Legal Remedies I understand and agree that violating this Agreement may cause harm to the Program. Lauren B’s Jr. Beauty Boot Camp may seek injunctive relief, monetary damages, attorney’s fees, and any other remedies allowed by law. 5. Acknowledgment I have read and fully understand this Agreement. I acknowledge that: These terms are legally binding. I had the opportunity to review this Agreement before signing. I voluntarily agree to comply with all terms indefinitely.
